Billing

• I am contracted with Medicare, United Healthcare, Colorado and Rocky Mountain HMO Medicaid, Aetna, Cigna and Anthem/BCBS and others. I will need a copy of the insurance card for billing purposes. I also accept cash pay if not contracted with insurance. Remember to communicate about any schedule changes to avoid no show fees.
